The New Business Tax Affidavit form (PDF) and supplemental Santa Clara Fire Department Permit Fee Assessment Form requires Adobe Acrobat Reader, which may be downloaded for free.

The forms may be printed, filled out, and mailed or delivered to the Municipal Services Division at the address listed above. The completed forms must be accompanied by a check for full payment of the required fees, as computed from the information you have supplied on the New Business Tax Affidavit.

New businesses: The completed and signed forms and fee payment must be received no later than thirty (30) calendar days after commencing business operations in Santa Clara, or a late penalty equal to 100 percent (100%) of the total fees due will be assessed. Along with the completed and signed Application and payment, individuals need to provide us with a copy of their "Fictitious Business Name" statement, Articles of Incorporation if they are a Coporation, Partnership or Limited Liability Corp, and sellers permit (if required) before we can process their application and issue them a City of Santa Clara Business License. Restaurants also need to provide a statement from the Department of Health. Incomplete or unsigned forms and/or incorrect fee payments will be returned, and may result in a late penalty. If you need assistance in computing the total required fees, contact the Municipal Services Division for assistance.

Business Certificate (License) Renewals: It is the responsibility of each business to renew their Business Certificate before it expires. However, as a reminder, a renewal application will be mailed to the business prior to the expiration date. If this tax is not paid before the expiration date, a penalty equal to 100 percent (100%) of the total tax will be assessed.

Separate forms must be completed for each location and for each separate type of business at the same location.

If your organization or corporation has been granted exemption from taxes, please include proof as provided by the Internal Revenue Service (Federal Form 501 C).

Businesses operating out of the home are required to come into City Hall for Planning Department approval; they are located in the Permit Center, across the hall from the Municipal Services Division.
