Special Transportation Permit (Oversize Load)

To transport wide loads or oversize loads on City streets, a permit is required.  Due to the requirement for an original signature, the PDF form provided here is not interactive but it may be printed and filled out, then mailed, faxed or hand-delivered to the Engineering Department at City Hall.


To obtain a permit using this form, you must complete the following:


1. Print out the Special Transportation Permit application form (PDF). 
2. Fill out the permit application form completely.  Include an authorized agent signature.
3. Mail, fax (number provided above) or deliver the completed permit application form, with payment (indicated on the form) to:

Allow at least two (2) working days to process your request for your wide or oversize load permit before contacting us.  Failure to provide complete information may delay issuance of the requested permit.


Fax Service
Businesses in good standing with the City of Santa Clara are entitled to use a special fax service.  Faxed permit forms must be accompanied by proof of payment, which may be in the form of a photocopy of the signed check made payable to the City of Santa Clara.  To remain in good standing, all checks for payment of wide or oversized load permits must be received by the City of Santa Clara within ten (10) working days of the faxed request.

Payment by Credit Card may be authorized by returning a Fax Authorization Form.
