Q: How do I file a claim against the City of Santa Clara?

Claim forms and instructions are available online and from the City Clerk’s Office, (408) 615-2220, 1500 Warburton Avenue, Santa Clara, California 95050. The claim form must be filed with the City Clerk’s Office within six (6) months from the date of the incident. The City Clerk’s Office will forward the claim form to the City Attorney’s Office. All claims are administered by the City Attorney’s Office and the City’s Third Party Administrator.
