Health and Wellness Program

Care Management

Care Management is a program that empowers older adults to live at home safely and independently by providing the information, support, and advocacy needed to make decisions and access resources. The care manager does an in-home assessment, helps to create an individualized care plan, coordinates services and offers ongoing support to older adults and their families. Residents of the City of Santa Clara, aged 60 and over, are eligible for care management services. There is no fee for services, and donations to the program are appreciated. Please call the Senior Center at (408) 615-3170 to schedule an appointment.

Health and Wellness Program

Santa Clara Senior Center wishes you Health and Wellness, and we are ready to help you achieve it!

The Health & Wellness Program provides preventative and community health nursing, free of charge, for Santa Clara city residents age 50 or older. Working closely with the Senior Center’s case manager, the program’s registered nurses provide assessment, teaching, and public health nursing. Visits are by appointment or during scheduled drop-in days.

What does the program provide?

  • Health information and education
  • Accurate monitoring of blood pressure and blood sugar values
  • Teaching about medications
  • Annual flu shot clinic
  • Links with community agencies
  • Safety checks for special needs
  • Healthcare and resource referral
  • Walk-n-Talk on Wednesdays for exercise and socialization

Is there a fee?

This is no charge to participate in the Health and Wellness program, with the exception of a small donation for a flu shot at our annual clinic.

Do I still see my physician?

Absolutely! Your physician directs your care; we help you follow those directions. Be assured that all medical information is confidential.

Can’t leave home?

Call to arrange for a home visit if your health limits your mobility.

Our goal is to assist you to achieve your personal best level of health and wellness. We also hope to provide some familiar faces with whom you can discuss your health care questions.

If you think we can help, we will be happy to provide an appointment with you for your particular concerns.

Registered Nurses are available Monday through Friday by appointment. Please call the Senior Center at (408) 615-3170 to schedule an appointment.

On-going Programs Offered at the Senior Center

Walk-n-Talk

Wednesday, 10 am 

A health awareness exercise and socialization program for seniors. Walk around the Senior Center and nearby park and then enjoy conversation with friends, new and old. This program is free and available to individuals over the age of 50.

Drop-in Blood Pressure Check
Thursday, 9:30 am - 11:30 am

To have your blood pressure checked at other times, please call to make an appointment at (408) 615-3170.
