Care Management

Care Management is a program that empowers older adults to live at home safely and independently by providing the information, support, and advocacy needed to make decisions and access resources. The care manager does an in-home assessment, helps to create an individualized care plan, coordinates services and offers ongoing support to older adults and their families. Residents of the City of Santa Clara, aged 60 and over, are eligible for care management services. There is no fee for services, and donations to the program are appreciated. Please call the Senior Center at (408) 615-3170 to schedule an appointment.

Ten ways the Care Manager can help:

1.    Food/meal programs
2.    Outreach paratransit
3.    Housing options
4.    Medi-Cal/In Home Supportive Services
5.    Home care referral
6.    Advance Health Care Directives
7.    Caregiver Support
8.    Adult Day Programs
