Mission City Connections is the new
name adopted to represent the Santa Clara Senior Center's health
and social services. These services are collaborating to bring
the seniors of Santa Clara even better service. Things that
MCC can do for seniors and their families include:

- Information & Referral

- Caregiver Consultations

Services included under MCC are:

| Nutrition Program |
This federally sponsored program,
which provides a well-balanced, hot meal each weekday at 11:30 am, is for those that are unable to prepare a nutritious
meal, those that need to cut the cost of food bills, and also
for those that would just enjoy some company while eating.
Meals are provided for those age 60 and older; a donation
of $2.00 per meal is requested. For meal
reservations, call the Nutrition Office at least two days in advance, Monday through Friday,
between 10 a.m. and 2 p.m. at (408) 615-3174.

| Home Care Registry |
Resource Specialists match home
care workers with clients requesting assistance in their homes.
Privately paid by the client, these workers are interviewed
by the registry and references are checked before being referred
to the senior. Services include personal care, companionship,
shopping, cooking, laundry, housekeeping, and driving. Hourly
rates start at $12.50 per hour for at least three hours, while
live-in care is available for a minimum of $110 per 24-hour
period.

| Health and Wellness Program |
A health assessment program, managed
by Registered Nurses, to be used as a supplement to the care
given by a senior's own private physician. The program can
help the well senior to monitor blood pressure or blood sugar
values, or to obtain information on medications, diet or lifestyle
changes. For homebound seniors, there are home visits and
telephone contacts meant to support their efforts to maintain
their levels of wellness at home. Other services include classes
on relevant topics, a lending library of health videos, and
an annual flu shot clinic.

| Social Services |
These services help seniors find
ways to deal with the day-to-day challenges of being an older
adult. These services are available to those age 60 and older,
that reside in Santa Clara or Alviso. Information and Referral,
individual and group counseling, and caregiver consultations
are available to persons living outside of Santa Clara as
resources allow.

| Case Management |
This program provides frail elders
with the opportunity to live as independently as possible
in his or her own home. By directing them to the many resources
in our community, the Case Manager counsels the family to
explore options and to develop answers for changing situations.
Services include a comprehensive evaluation, care plan, linkage
with community resources, and continuous progress monitoring.

| Caregiver Consultations |
Individuals and families are provided
with personal guidance, information, and support regarding
caregiver problems.
|
| Telephone Outreach |
Volunteer staff is available to
make telephone contact with homebound elderly people. These
calls provide "chat" time as well as a chance for
us to monitor needs.

| Information and Referral |
| This service helps seniors to
access and link up with the many community resources available
to the elderly and their caregivers.
|
| Support Groups |
Each year, we hold several courses
for caregivers, in which we cover such topics as care planning,
decision making, relationship issues, stress management, health
issues, legal and financial concerns, and placement options.
In addition, there is a monthly support group sponsored by
the Alzheimer's Association for caregivers of family members
with dementia.

| What Does It All Cost? |
We rely on multiple sources for our
funding, a third of our program being subsidized by the Council
On Aging of Santa Clara County. The balance of our operating
budget comes from Santa Clara Senior Center fundraisers, grants,
and private donations. For that reason we ask for financial
help from those who personally benefit from the program. While
there is no set fee, we do request a donation commensurate with
the type of service given and one's ability to pay. Regardless,
cost will not be a barrier to receiving services.
