Our Health & Wellness Program promotes health and independent living by connecting Santa Clara seniors, ages 50+, to services, information, and activities. The Programs Registered Nurses, in partnership with the Senior Center's Care Manager, work on situations requiring an in-depth assessment or on-going support.

ID Fit Program 

Santa Clara residents under the age of 50 with a qualifying disability or medical condition may utilize the Fitness Center and Natatorium. A completed Registration and Release of Liability Form is reviewed by the Health & Wellness staff for approval. Health Education and Coaching

  • Learn information about health issues that can affect your independence.
  • One-on-one coaching to determine your personal health priorities and goals. 
  • Classes and groups addressing current health trends and topics.

Support Groups/Workshops

Our support groups teach you the tools to strengthen and enrich your life, whether you're coping with loss, trying to decide what retirement means, or just looking to climb out of a rut.  Various support groups and workshops are offered throughout the year that address topics such as anxiety, stress reduction, grief, and relationships. These groups change on a regular basis and space may be limited. No registration is required. Care Management Services

Care Management empowers older adults to live at home safely and independently.  The care manager provides information, support, and the advocacy needed to make decisions about care and to gain access to resources.  The care manager assesses needs, helps create an individualized care plan, and offers ongoing support to older adults and their families.

Who is eligible?

Santa Clara residents, age 60+, and their family members.

What is the cost?

Care Management services are provided by the City of Santa Clara with no fee to clients.

  • Our efforts to serve seniors are strengthened significantly through collaborations with a variety of individuals and organizations.


Donations are accepted and are used to continue our high quality programs.  You can even donate to a specific program of your choice by indicating the program you want to benefit from your donation.

Donations of $100 or less are accepted at the Senior Center.  Donations to the Senior Center in excess of $100 are presented for acceptance to the Santa Clara City Council.

Monetary donations can be mailed to 1303 Fremont Street, Santa Clara, CA 95050.  Checks should be made payable to the City of Santa Clara.
