Our Health & Wellness Program promotes health and independent living by connecting Santa Clara seniors, ages 50+, to services, information, and activities. The Programs Registered Nurses, in partnership with the Senior Center's Care Manager, work on situations requiring an in-depth assessment or on-going support.

Care Management Services

Care Management empowers older adults to live at home safely and independently.  The care manager provides information, support, and the advocacy needed to make decisions about care and to gain access to resources.  The care manager assesses needs, helps create an individualized care plan, and offers ongoing support to older adults and their families.

Who is eligible?  Santa Clara residents, age 60+, and their family members.

What is the cost? Care Management services are provided by the City of Santa Clara with no fee to clients.
