Presented by Community Health Partnership (CHP), founded in 1993, is a consortium of  ten community health centers in Santa Clara and San Mateo Counties serving 181,000 patients annually. These include federally qualified health centers, school health clinics, free clinics, reproductive health care clinics, and an Indian Health Center.
