Fire Department Public Records, Incident, and Investigation Report Requests
Requests for fire department public records, fire incidents and fire investigation reports for incidents, buildings or property within the City of Santa Clara can be requested using the NextRequest web portal.  To access the City of Santa Clara NextRequest web portal, please click the following link or copy and paste the link into your browser: https://santaclara.nextrequest.com/

Requests submitted through the NextRequest web portal will be routed to the appropriate department or division for review and processing.  Upon approval, you will receive an email from NextRequest with a link to retrieve the requested information.

Fire Department Subpoena Requests
Subpoena for fire department records can be submitted via email at Fire@SantaClaraCA.gov or via postal carrier to:

Pre-Hospital Care Reports for EMS Calls

Pre-Hospital Care Reports (PCRs) for EMS calls are available upon direct request to the Fire Department. These reports may contain sensitive or personal information, and as such, are only releasable to the patient, the patient’s legal representative or the patient’s guardian. An Authorization for Medical Release form must be completed and submitted with the report request.  Reports can be picked up in person or sent via email.  If you are requesting the report to be emailed, a Request for Electronic Transmission of Personal Health Information form must also be completed and submitted (can be submitted with the Authorization for Medical Release).

Please note that government issued photo identification will be required at the time of report request.

Please complete the required form(s) in their entirety and email to: Fire@SantaClaraCA.gov
Forms can also be submitted in person to the City of Santa Clara Fire Administration Division, located at 777 Benton St., Santa Clara, CA 95050.  Public hours are Monday – Friday from 8:00 a.m. to 5:00 p.m., excluding City holidays.  For a faster turnaround, we encourage electronic submittals.
 
Ambulance Billing Requests or Inquiries

Billing requests or inquiries for ambulance transports provided by the City of Santa Clara Fire Department can be requested by email at Fire@SantaClaraCA.gov

Please provide the patient’s name, date of service and incident or report number provided at the time of transport. You may be asked to complete the Authorization for Medical Release form, depending on the request. These can be requested along with the PCR request.
